      The Transition Coordinator plays an essential role in supporting individuals as they take steps toward discharge from the Foundation Intensive Outpatient Program (IOP). Collaborating with a multi-disciplinary team, the Transition Coordinator ensures that the care provided meets the needs of clients within the IOP. Specifically, the Transition Coordinator considers clients’ emotional, psychological, vocational, social, physical, and spiritual needs, and develops a comprehensive after-care plan.

      Under the direction and supervision of the Intensive Outpatient Program (IOP) Coordinator, the Behavior Health Associate – IOP Transition Coordinator is an in-training therapist who assists with the admission of patients into the program, staying connected with existing Therapeutic/Psychiatric resources the patient currently has, coordinating additional community-based care unique to the patient’s needs, and providing education to the patients to help ensure successful discharge of the program. As a member of the Foundations IOP, the Behavior Health Associate – IOP Transition Coordinator assists with assessing behavioral health needs of patients enrolled in the program (including emotional, psychological, cognitive, social and biological factors) that shape an individual’s well-being. Under the direction and supervision from the IOP Coordinator and Treatment Team, this position provides interventions that focus on the member’s contextual factors and through behavioral health interventions works to improve the member’s recognition, treatment, and management of their health conditions. This clinical work will occur primarily in-person but may have components of virtual care or via telephone. The Behavior Health Associate – IOP Transition Coordinator will also assist with, or facilitate, psychoeducational groups during program hours. These groups will predominately revolve around patient discharge planning, and the importance of ongoing care, but may extend into other pertinent domains of the patient’s overall well-being. Additionally, the Behavior Health Associate – IOP Transition Coordinator will participate in weekly staffing meetings to discuss each patient’s progress in the program, provide clinical input into direction of care, and keep the treatment teams well informed on the patient’s readiness for discharge. Thorough documentation from the Transition Coordinator will be recorded in the patients Electronic Medical Record (EMR).

      JOB SPECIFIC MINIMUM QUALIFICATIONS

      1. A Master’s degree in Social Work, Professional Counseling, Marriage and Family Therapy or related field from an accredited graduate program.
      2. Valid, State of Wisconsin license as an Advanced Practice Social Worker, LPC-IT, MFT-IT or above. Applicants demonstrating significant, specific clinical practice training in a PCBH program may be considered if licensure is obtained within three months of their start date.
        1. If the Transition Coordinator has any of the following licenses (APSW, LPC-IT, LMFT-IT) they will have clinical supervision and additional reporting responsibilities to a Clinical Supervisor. This Clinical Supervision will be provided to aid in the Transition Coordinator’s goal of professional licensure.
      3. Possession of current State of Wisconsin Driver license with a good driving record, access to a vehicle, and the ability to travel.
      4. An understanding of the relationship between medical and psychological aspects of health and disease, and the ability to apply the bio-psychosocial model to treatment in the primary care setting.
      5. Knowledgeable of and skilled in the provision of evidence-based mental health therapy services including assessment, diagnosis and treatment of individuals and families.
      6. Interest in learning evidence-based, brief interventions that emphasize patient functioning, and a commitment to continued skill development as a behavioral health professional.
      7. Ability to provide culturally competent services and to work effectively with persons of diverse cultural backgrounds.
      8. Ability to occasionally work up to one evening per week and attend meetings outside of clinical hours as required (infrequent).
      9. Outstanding customer service skills: Ability to be culturally sensitive and approach customers in a non-intimidating and supportive manner, and the ability to represent GHC-SCW in a professional manner.
      10. Knowledge of and willingness to learn community resources and referral sources appropriate for member population groups, and the ability to work closely with community partners in a professional and respectful manner.
      11. Knowledge of and ability to learn aspects of the Medical Assistance, BadgerCare programs, and GHC-SCW’s related services and benefits.
      12. Access to and ability to use a personal computer with stable access to the internet and video conferencing capacity.
      13. Knowledge of or ability to learn and use various computer programs including Microsoft business applications and EPIC software.
      14. Excellent verbal and written communication skills.
      15. Ability to maintain patient confidentiality required. Knowledge of and ability to learn HIPAA requirements.
      16. Excellent organizational and time-management skills.
      17. Ability to actively listen, problem-solve, and be flexible as a member of a team.
      18. Ability to work efficiently and under stressful situations.
      19. Ability to work respectfully and cooperatively with members of the public and all GHC-SCW staff.
