Serve as Behavioral Health’s “Care Traffic Controller,” working closely with patients’ PCP and other devoted teams to facilitate and track resolution of clinical needs, such as locating in network behavioral health services, scheduling appointments, collaborating with members to secure ongoing care in their community and medical record requests
Assist physicians, nurse practitioners and social workers in reviewing patient panel needs and expediting/prioritizing key care coordination tasks
Monitor new referrals and completed telephonic patient outreach and engagement
Telephonic patient check-ins
Prepare for, and actively participate in, weekly Interdisciplinary Team Meetings, helping the team to identify high risk patients, resource needs, and next steps
Provide continuous administrative and outreach support for patients transitioning from Devoted’s Behavioral Health team to longer term care in their community, ensuring safe, smooth transitions
Coordinate relationships with community behavioral health partners including locating and confirming in network and community based providers and services as well as performing outreach and engagement with these services
Highly organized and detail-driven
Works well in a fast-paced environment with firm deliverables and time frames
Curiosity to recognize process deficiencies, recommend improvements and partner to implement solutions
Excited to be an active participant and do what it takes to achieve fast-paced forward progress
Preference toward collaboration and preventing silos; be a team-player and able to work in a cross matrix setting
You are comfortable working with patients by phone
Agility and collaboration are critical to your success - we are a new organization and have a start-up mentality
You are comfortable learning new technologies including electronic health records and different computer platforms, operating systems and programs (Google Chrome, Google Sheets)
You have a passion for supporting the delivery of healthcare that we would want for our own family
You enjoy a fast-paced, high-energy, growing organization
Prior experience working in a behavioral health setting
Bilingual preferred (English/Spanish) but not required
Geriatric experience or experience coworking with a Medicare population
Health insurance experience, particularly Medicare Advantage
Prior experience working with complex patients and/or underserved populations
Bachelors degree or in health sciences or related field and 3 years’ work experience in a healthcare setting
Two (2) years of experience in behavioral health
The ability to comfortably multi-task: you’ll be listening, talking and typing all at the same time
Team player mentality with a can-do attitude
Comfortable working with internal and external stakeholders to advocate for our patients

Devoted Medical was founded on the belief that if we treat each member like we would our loved ones, we can improve healthcare experiences and outcomes for our patients. The Behavioral Health Program is at the heart of this goal.
Devoted Medical has designed a Behavioral Health program to manage and coordinate care in a manner consistent with Devoted’s values of treating every member like family. Members with Behavioral Health conditions often lack access to high-quality care because of lack of access to community resources or personal barriers associated with their behavioral health needs that prevent them from following through to seek care.
We are on a mission to provide well coordinated behavioral health care to Devoted members. Our team combines short term intervention alongside seamless care coordination to support our members securing, and staying committed to community behavioral health intervention.
As a Behavioral Heath Care Coordinator you will have the opportunity to provide care coordination services to members referred for Behavioral Health support.

Salary Range: $22-$27 per hour
