What to Expect (Job Responsibilities):
- Conduct initial, annual, and as needed face-to-face assessments
- Provide advocacy and service coordination for individuals with SPMI
- Assist in eligibility determinations
- Work closely with providers and community organizations
- Perform prior authorizations for services
What is Required (Qualifications):
- Graduate degree in social work, psychology, or other behavioral health science field
- Qualified Mental Health Professional (QMHP) designation
- 2 years of experience working with individuals with severe and persistent mental illness
- Reside in the geographic area with willingness to travel up to 50% of the time
- Licensed by the State of Oregon
