      • Schedule: Monday-Friday 7:00 pm - 3:30 am; Rotating weekends

      • Location: Hoffman Estates, IL
      • Salary: $28.88 - $42.23 per hour

      Responsibilities

      Coordinate comprehensive plan of care for patients.

      • Oversee primary needs of patient, including assessment of client and family/caregivers, and develops comprehensive plan of care.
      • Provide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with physician.
      • Monitor patient care through assessments, evaluations and/or patient records.
      • Advocate for resources and removal of barriers.
      • Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

      Requirements

      Licensure / Certification / Registration:

      • Counselor credentialed from the Illinois Department of Financial and Professional Regulation preferred.
      • Social Worker credentialed from the Illinois Department of Financial and Professional Regulation preferred.
      • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

      Education:

      • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.

      Additional Preferences

      **Master's degree in social work, psychology, or related field

      **Experience in completing Crisis assessments is preferred
