- Conducts program orientations individually with consumers;
- Explains the structure and requirements of the JAG Restorative Justice Partnership and Diversion; advises consumers of their rights, responsibilities and eligibility for program participation; explains rules, regulations and policies to consumers;
- Conducts in person interviews with consumers to complete intake information which includes consumer's social, medical, psychological, substance use, legal, financial and personal history;
- Assesses consumers’ individual needs, identifies appropriate resources, helps coordinate and facilitate linkage to appropriate resources, and supports consumer engagement through the use of strengths-based service delivery and modeling of restorative justice practices;
- Participates in multidisciplinary team meetings; discusses consumer observations and consumer progress with professional staff as input to treatment review;
- Works collaboratively with diverse teams, including partners with the District Attorney’s office, Public Defender’s office, Probation, community providers, and healthcare providers;
- Attends court proceedings with consumers, if needed;
- Supports consumers with participating in Restorative Justice Conferences and completing agreement items, as necessary;
- Writes case notes and prepares other appropriate documentation in both written and electronic formats including the AVATAR program; prepares, maintains and provides program-related written information, statistics, and tracking spreadsheets and reports;
- Makes referrals to and/or assists consumers in obtaining supportive services including services to meet basic needs such as safety, food, shelter, clothing, medical care, etc;
- Monitors consumer progress with treatment plan and adjusts plans as necessary; sets up and maintains controls to ensure timely actions and quality case management;
- Engages participants in human-centered conversations through the use of motivational interviewing techniques, trauma-informed care, and cultural humility;
- Observes attitudes and behaviors to help consumer identify consistencies and confront inconsistencies, identifies and addresses barriers that may prevent consumer from obtaining/maintaining recovery and/or self-sufficiency; counsels consumers to encourage their responsibility in achieving realistic goals and successful completion of their legal diversion case;
- Teaches consumers life skills by assisting with banking, grocery shopping, household budgeting, hygiene, etc.; coordinates, facilitates and participates in group outings for the purposes of community integration and education;
- Visits consumers in home or other community locations as necessary; transports consumers as necessary; calls consumers with appointment and/or medication reminders; AND
- Provides conflict resolution in emergency/stressful situations requiring immediate attention.

- Monitors consumer compliance with treatment plan; sets up and maintains controls to ensure timely actions and quality case management.
- Observes attitudes and behaviors to help consumer identify consistencies and confront inconsistencies, identifies and addresses barriers that may prevent consumer from obtaining/maintaining recovery and/or self-sufficiency; counsels consumers to encourage their responsibility in achieving realistic long-term sobriety and/or behavioral health goals.
- Teaches consumers life skills by assisting with banking, grocery shopping, household budgeting, hygiene, etc.; coordinates, facilitates and participates in group outings for the purposes of community integration and education.
- Attends court proceedings with consumers or to testify if subpoenaed.
- Visits consumers in home or other location as necessary; transports consumers when necessary; calls consumers with appointment and/or medication reminders.
- Provides conflict resolution in emergency/stressful situations requiring immediate attention.
- Performs outreach and community engagement functions such as promoting and providing special presentations to outside agencies, schools, and community groups and acting as a liaison with local community and law enforcement agencies.

The County of Yolo Health and Human Services Agency (HHSA) is recruiting to three (3), full-time positions in the class of Behavioral Health Case Manager I/II. Two (2) position will be permanent status and one (1) will be a limited term position with a projected end of 6/30/2026. The level of hire will be determined by the hiring manager at the time of offer.
One (1) position will be with the Homeless Services Team which specializes in providing intensive wrap-around services to medically vulnerable individuals experiencing homelessness. Our team offers trauma-informed care to those with complex and chronic health conditions, while also conducting outreach and linkage services for unhoused individuals throughout the county.
This is a field-based role, where case managers regularly meet clients at shelters, encampments, and other locations. They also transport clients in county vehicles, accompanying them to medical appointments, court, treatment centers, and other necessary destinations. Previous experience in field outreach is preferred, as is familiarity with the Homeless Management Information System (HMIS).
The ideal candidate will:
AIC utilizes a similar model to the successful Yolo County Mental Health Court program. Participants progress through four phases: 1. Engagement, 2. Transition, 3. Sustainability, and 4. Maintenance. Progression through the four phases includes increasing days of sobriety, drug and alcohol testing, participation in a 12-step treatment program or an equivalent program, obtaining employment and permanent housing, and decreasing the number of arrests. Program graduates are required to remain arrest-free for a minimum of 9 months, maintain a minimum of 180 continuous days of sobriety, and establish an exit plan with the program's clinical team and Probation Officer.
The program is completely voluntary; AIC offers individuals who want to seek treatment for substance use disorder concerns a support system to help accomplish that goal. AIC uses an incentive-based system to encourage progression through the four phases, which may include, but are not limited to, incentives, reduced reporting to the Probation Officer and AIC team, reduction of fines and fees, letters of recommendation for employment, and potentially the dismissal of charges upon successful completion of AIC and probation (at the discretion of the AIC team).
This Behavioral Health Case Manager will support individuals participating in AIC. The ideal candidate will have:One (1) position will be assigned to Justice Assistance Grant/ Restorative Justice Partnership- Case Managed (RJP-CM) team. RJP-CM is a legal diversion program with a strong focus on restorative justice, addressing the root causes that contributed to legal involvement, and developing pathways to mitigate recidivism.
The incumbent will primarily serve identified program participants living with mental health conditions, substance use disorders, and/or significant trauma, who may be unhoused or living in precarious environments. Meeting with program participants in community and field-based settings is a requirement of this position, as is travel to surrounding counties using a County vehicle.

Definition
Under supervision, performs paraprofessional tasks related to the treatment of consumers with behavioral illness, emotional disturbance, and/or substance use disorders related to the behavioral health illness or emotional disturbance; provides liaison between professional staff, consumers and community agencies; and provides referral, advocacy, and support services.
Distinguishing Characteristics
The Behavioral Health Case Manager I/II is a single class with two salary levels, and positions in this class are flexibly staffed. Incumbents provide case management services related to the consumer's behavioral health service needs, including but not limited to linkage, referral, advocacy, monitoring signs and symptoms of behavioral health stability as directed by a licensed/waivered clinician, licensed medical staff, clinical manager, etc.
The Behavioral Health Case Manager I is the entry level in this class series. Under close supervision, incumbents are given detailed instructions in the performance of routine tasks that are more structured and repetitive than those assigned at Level II. As requisite skills and knowledge are developed, incumbents are expected to perform increasingly responsible and difficult assignments. Incumbents may advance to the higher level after demonstrating the ability to perform the full journey level duties of the class, which generally requires twelve months of experience at Level I.
The Behavioral Health Case Manager II is the journey level in this class series. This is the targeted competency and performance level of all Behavioral Health Case Managers. Working under general supervision, incumbents are expected to be proficient in performing a variety of responsible and difficult assignments. This class is distinguished from the next higher classification in the job series, Behavioral Health Case Manager III, in that the latter class is the advanced journey and lead level classification requiring the incumbents to possess a Mental Health Rehabilitation Specialist (MHRS) or Certified Alcohol Drug Counselor II (CADC-II) designation. Duties may include, but are not limited to, the following:
Education: Graduation from high school or possession of a valid G.E.D. certificate; AND
Experience: One (1) year of full-time, paid or volunteer work experience in a behavioral health treatment setting requiring extensive public contact.
Behavioral Health Case Manager II:
Education: Graduation from high school or possession of a valid G.E.D. certificate; AND
Experience: Two (2) years of full-time, paid or volunteer work experience in a behavioral health treatment setting requiring extensive public contact, including at least one (1) year of experience comparable to the class of Behavioral Health Case Manager I in the County of Yolo.
Note: Proof of registration of certification as an Alcohol & Drug Counselor, pursuant to 9 CCR § 13000 et al is required by positions within this class performing substance use duties, in addition to the education and experience requirements.
License
