      The Behavioral Health Caseworker is responsible for coordinating services and addressing social determinants of health for Members with behavioral health conditions. Under the direction of the Population Health Manager and Supervisor the Behavioral Health Caseworker ensures the provision of services in the least restrictive setting and transitions support across and between care settings; connects members to services that promote community living and helps to overcome any barriers to health; establishes points of contact in order to collaborate with identified community, medical and/or behavioral health teams.

      The Behavioral Health Caseworker meets with Members to whom they’re assigned in the home or least restrictive community setting. They will develop an understanding of that Member’s specific circumstances and needs, assess what services are available and create a person-centered plan for moving ahead in a way that will best resolve the Member’s behavioral health needs. For all physical health needs the case worker will consult with a registered nurse care manager.

      The Behavioral Health Case Worker will:

      • Support Members during behavioral health transitions through person-centered assessment and coordination of resources. The caseworker collaborates with Members during times of transitions, provides , referrals and linkage to community resources while continually evaluating the person -centered plan.
      • Review medication list with members and coordinate pharmacy needs to support medication adherence.
      • Monitor, and update the person-centered plan through regularly scheduled follow-up contacts preferably in the Member’s home. The plan should always be evaluated based on the Member/caregiver progress, needs and preferences.
      • Establishes points of contact in order to collaborate with identified community, medical, and/or behavioral health teams.
      • Maintain timely, complete, and accurate documentation of Member interactions in ACFC electronic care management platforms where applicable.
      • Develop a working knowledge of ACFC electronic care management platforms, care management programs, policies, standard operating procedures, workflows, Member insurance products and benefits, community resources and programs, and applicable regulatory, state, and NCQA requirements.
      • Support a positive workplace environment, collaborate, and share knowledge and skills to support our culturally and demographically diverse Member population.
      • Conduct Face-to-face visits at the Member’s residence, provider’s office, hospitals, other acute location or community location for education and/or assessment.
      • Other duties as assigned.

      Education/Experience:

      • FIELD-BASED REMOTE to qualified residents living in South Carolina.
      • Required Bachelor's Degree in Human Services (i.e., social work, psychology, sociology, early childhood education, child development, criminal justice) or related field. Masters prepared will be given preferred consideration.
      • 2 years of proven professional experience working directly with SMI members (Serious Mental illnesses) aged six and up who may have one of the following conditions: Psychotic Disorders, Bipolar Disorders, Major Depression.
      • 2 years of social services experience, engaging with clients (e.g., individuals, families, communities), routinely assessing their needs, connecting them to services, and monitoring their progress. Experience working with the Foster Care population is an added plus.
      • Proficiency using MS Office (Word, Excel, Outlook), internet applications, and electronic medical record and documentation programs. Electronic Case Management Platform.
      • Must be able to manage a large caseload of up 60 members and support during transitions of care through assessment, coordination of care, education of the discharge plan of care, referrals, and evaluation of the plan's effectiveness.
      • Strong documentation and record-keeping skills are required, emphasizing accuracy and adherence to established methods.
      • Proven professional expertise in assessing members' situations, developing care plans, and teaching self-management in assessing member's situation, developing a care plan, and teaching self-management. Skilled in working one-on-one with members, effectively communicating preventative health and management philosophies, and presenting health education materials to both individuals and small groups.
      • Required travel and willingness to access members' home and community locations within assigned regions. Must possess reliable transportation, a valid driver's license, current auto insurance, and willingness/ability to drive a personally owned vehicle.
