- This is a remote field-based position requiring travel within Delaware, mainly Kent and Sussex Counties.
- Coordinating Member care needs with BH Providers
- Developing processes to coordinate behavioral healthcare between Primary Care Physicians and Behavioral Health Providers
- Participating in identifying best practices for BH in a primary care setting
- Coordinating behavioral care with medically necessary services
- Being knowledgeable of the PROMISE requirements and coordinating with PROMISE providers to effectuate the requirements
- Knowledge of and experience with behavioral health and substance use disorder providers
- Develop and maintain relationships with behavioral health and substance use disorder providers
- Master’s Degree required
- 3-5 years prior experience in behavioral health/human services required; or other equivalent background and experience that would translate well to this position
- Active Delaware Licensed Clinical Social Worker (LCSW) required
- Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint); MS Access is a plus
- Ability to travel up to 25% throughout the state of DE
- Valid driver’s license
Role Overview
The Behavioral Health Coordinator is responsible for acting as a liaison and support person between the Division of Medicaid and Medical Assistance (DMMA) and the Case Management team, the Division of Substance Abuse and Mental Health (DSAMH), and Behavioral Health providers. The Coordinator will also be responsible for outreach activities with members and providers and documenting all activities in the electronic case management documentation system.
