Job Description

- Support our clients (NDIS participants) in person, along with their families and/or carers, to identify their needs through assessment and person-centred planning
- Identify meaningful goals and practical strategies utilising The Positive Behaviour Support Capability Famework, with a commitment to eliminate restrictive practices, in the home and community
- Develop, deliver and evaluate evidence-based support plans within a complex support space
- Ensure individual plans reflect our clients' goals, passions and strengths
- Build the knowledge, capacity and strengths of those who support our clients - family, carers, friends, etc

- Current Drivers Licence and own reliable vehicle
- Degree level qualifications of relevance to providing supports to people with behaviours of concern are highly preferred
