Job Summary
The Lead Scanning/Record Completion Assistant, HIMS Record Completion Unit is responsible for proving training and lead to AAI and AAII with scanning ,tracking, validating, retrieving, and analyzing medical documentation for the Medical Staff. This position performs daily and other scheduled functions using Chart Fact/Locator/Reserve/ID/Script systems, to review and analyze incomplete medical records.
DUTIES AND ESSENTIAL JOB FUNCTIONS
Coordinates and assists processing functions for all requests for information from all requesters including subpoenas and court orders.
A. Screens and verifies requests for information from all requesters in compliance with state and
federal regulations and hospital policy.
Verifies patient information, service dates, and obtains medical record numbers.
Enters all requests into the Correspondence Tracking Module.
Forwards requests to appropriate department for processing when necessary.
Generates and mails letters on request status, as appropriate.
B. Retrieves and processes requested medical record information from appropriate media.
Reviews contents of record to ensure completeness. Obtains any documents not filed in the medical record as needed.
Escalates issues, concerns and opportunities for quality and efficiency improvement to management
C. Verifies authorization in compliance with state and federal regulations and hospital policy.
Returns any authorizations that are determined to be invalid back to the requesting party.
D. Assists outsourced service with requested medical record appropriately to complete request:
Personally copies or faxes requested information as needed.
Assists copy service to locate correct documentation for completion.
Coordinates receipt of witness/retrieval fees, notification, and completion of requests and subpoenas, etc. for outside copy agencies retained by attorneys and other requesters per policy and procedure. (This may involve assisting the copy service.)
E. Performs follow-up as needed, until request is logged as complete in the Corresponding Tracking
Module.
Submits fees, money and checks received to the Hospital cashier, and routes the receipt to the department manager
Monitors and maintains the accuracy and timeliness of release of information functions.
Performs routine QA on the Copy Service Process
Recommends solutions for quality and timeliness improvement to management.
Performs other duties as assigned.

An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.
For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

Required Qualifications
1. 2 years recent experience as HIMS lead in a hospital setting
2. Medical Terminology course of study
3. High school graduation
4. Proficiency with Microsoft, EPIC and EPHI systems
5. Knowledge of repair of desktop , fax and copy machines
6. Aptitude for technical perception and analysis of information in different systems
7. Excellent interpersonal communication skills.Preferred Qualifications
1. Database knowledge and experience.
2. RHIT accreditation
