      Job Objective:

      Under the supervision of the Director of Health Information Services is responsible for ensuring the accuracy and completeness of cancer registry data; cancer registry system management; coding and abstracting; and participation in cancer program activities.

      Duties and Responsibilities:

      1. Accuracy and Completeness of Cancer Registry Data
        1. Designs and uses audit tools to monitor the accuracy of cancer registry data.
        2. Collects, compiles, analyzes, and presents cancer registry data.
        3. Assists clinicians and other researchers with in cancer related studies and research, providing guidance in the use of the cancer registry data system.
        4. Performs, participates in, and tracks data quality audits identifying areas for improvement.
        5. Develops action plans, revises guidelines, and educates based on the results of audits and changes in cancer registry standards.
        6. Provide data and education to hospital staff and medical staff relating to documentation requirements from the American College of Surgeons (ACoS).
        7. Keeps abreast of regulatory changes affecting Tumor Registry information required by the American College of Surgeons Commission on Cancer, the State of California, and others, as appropriate; and applies current approved definitions for coding and abstracting.
      2. Coding and Abstracting System Manager
        1. Serves as Cancer Database Liaison and as the primary resource for cancer registry system by performing advanced system management functions in a timely and accurate manner.
        2. Manage reporting tools (Rapid Quality Reporting System and Cancer Program Profile Reports) to benchmark performance and improve outcomes.
        3. Monitors system to ensure performance and i dentify deficiencies. Resolves or communicates with information systems department or vendor for corrective action.
        4. Maintains knowledge of and documentation of the database and definitions.
        5. Participates in updates and upgrades of system and revises affected processes.
        6. Completes submission of cancer registry data to the state in a timely manner.
        7. Trains staff on system functions.
      3. Coding and Abstracting
        1. Abstracts non-analytic and analytic cancer cases recording complete treatment information according to American College of Surgeons (ACoS), Commission on Cancer (CoC) and California Cancer Registry (CCR) guidelines.
        2. Reviews and screens the entire medical record to abstract designated statistical data and enters the information in the abstract database in a timely and accurate manner.
        3. Identifies cancer cases by referencing primary and secondary sources of information, including the facility’s diagnostic index and copies of pathology reports generated by the facilities’ pathology department.
        4. Codes topography, morphology, and other information relative to reportable cancer cases.
        5. Enters cancer registry information into database according to established procedures and regulations.
        6. Annual follow up on each known living patient accessioned into the registry from completion of treatment to death to determine their cancer status, quality and length of survival, subsequent treatment, and complete death information.
      4. Cancer Data Quality
        1. Responsible for ensuring standardization, quality and completeness of all cancer data in the cancer registry in compliance with state and national guidelines.
        2. Identifies key issues and patterns from partial/conflicting data and provides solutions for the same.
        3. Assists in identifying problems and opportunities for improvement in processes/workflows to be brought to Cancer Committee for review, problem solving discussions and resolution.
        4. Responsible for cancer data requests and analysis of requested data to ensure data accuracy.
        5. Coordinates QA functions with physicians to meet cancer registry data quality standards.
      5. Coordination of Day-to-Day Workflow
        1. Plans, organizes, maintains, and coordinates work to achieve cancer registry goals and maximum productivity and efficiency on a day-to-day basis.
        2. Provides technical and administration assistance in staff training, budget related processes, productivity management, recruitment, supply management, procedures and guidelines, special projects, shared folder maintenance and documenting performance of staff.
      6. Participation in Cancer Program Activities
        1. Prepares supporting documents and reports for Cancer Committee and attends meetings.
        2. Serves as liaison with community groups regarding cancer screening or education programs.
        3. Prepares annual hospital cancers reports which include illustrative graphs and charts of cancer cases reported with survival rates.
        4. Ensures that American College of Surgeons (ACoS) accreditation requirements are met.
        5. Interacts with department staff, hospital staff, medical staff and managers to ensure prompt and accurate communicating as needed to achieve Cancer Program goals.
        6. Provide guidance, data, and education to hospital staff and medical staff in relation to accreditation standards.
      7. Cancer Registry Review and Case Finding
        1. Identifies 98-100% of cancer cases according to Commission on Cancer (CoC), California Cancer Registry (CCR) and Surveillance, Epidemiology and End Results program (SEER) guidelines to be entered into the database. Screens and analyzes in-coming pathology reports, cytology reports, discharge summaries and other source documents for case-finding purposes and quality assurance.
        2. Obtains follow-up information from physicians and patients regarding treatments and length of survival.
        3. Generate and maintain worklists from the Cancer Registry system in a timely manner.
      8. Participation in Cancer Program and Cancer Conference Activities
        1. Develops agenda, support documents and coordinates cases for Cancer Conferences as needed.
        2. Coordinates with radiology, pathology, physicians and outside facilities to ensure all patient records needed for conference are available as needed.
        3. Coordinates all audio-visual needs for Cancer Conference as needed.
        4. Serves as Cancer Conference Moderator.
        5. Manages Cancer Conference attendance as needed.
      9. Other Duties
        1. Assists hospital staff, medical staff, public, and patients.
        2. Plans, organizes, prioritizes, and coordinates work to achieve functional area goals and maximum productivity and efficiency on a day to day basis.
        3. Answers telephones consistent with Service Excellence standards, addresses and/or routes calls appropriately and in a timely manner.
        4. Protects the confidentiality of primary and secondary health records and releases information as mandated by law, professional standards, and the hospital's policies.

      Non-Essential Duties:

      Accepts and carries out other miscellaneous duties and responsibilities as required for maintaining the services provided by Medical Records.

      Knowledge, Skills and Abilities:

      Knowledge

      · Knowledge of Microsoft Office applications.

      · Knowledge of database applications and spreadsheet design.

      · Knowledge of report writing software.

      · Knowledge of department policies, practices and operations.

      · Comprehensive knowledge of medical diagnostic and procedural terminology and abbreviations.

      · Understanding of disease processes, anatomy and physiology as necessary for assigning accurate codes.

      · Knowledge of federal and state regulations and requirements cancer registry information.

      · Knowledge of Cancer Coding and Abstracting functions normally acquired through a minimum of 3 years of Acute Care Cancer Registry Department experience.

      Skills

      · Reads handwritten and transcribed documents in the medical record and interprets information.

      · Knowledge of methods of coding and staging using American Joint Committee on Cancer (AJCC), International Classification of Diseases for Oncology (ICD-O), Surveillance, Epidemiology, and End Results program (SEER), and Extent of Disease (EOD).

      · Correlate generalized observations and symptoms to a stated diagnosis to assign the correct code.

      · Perform data entry accurately with use of a keyboard and mouse.

      · Perform duties using computers for coding, abstracting, and related research.

      · Effective oral and written communication skills.

      · Detail oriented and organized.

      · Strong analytical and problem-solving skills.

      · Proficient in using cancer registry software and data management systems.

      Education and Experience:

      Education

      • High school graduate or GED equivalent.

      · Completion of a 2-year accredited program for Oncology Data Specialist (ODS) with current affiliation with the National Cancer Registrar Association (NCRA).

      Experience

      · Five years of recent Cancer Registry experience in an Acute Care facility required.

      · Five years’ experience with cancer registry database, reporting, and data submission processes

      · required.

      · Three years’ Cancer Registry abstracting experience required.

      · Three years’ experience required with evaluating Cancer Registry and CoC standards, formulating action plans and working in collaboration with the Medical Staff and other Cancer Program participants to ensure goals are met.

      · Three years’ experience in identifying and resolving data edit discrepancies and errors required.
