What to Expect (Job Responsibilities):
- Establish trusting relationships with members, caregivers, clinic staff, and physicians
- Proactively engage members to coordinate their care needs
- Serve as a liaison between members, caregivers, and health services
- Conduct Social Determinants of Health assessments to identify member needs
- Enter timely and accurate data into the electronic medical record
What is Required (Qualifications):
- High School Diploma / GED
- Must be 18 years old or older
- 6+ months experience in a Call Center environment
- 4+ years of experience in customer service support and/or engagement
- Proficient computer skills, including Microsoft Word, Excel, Access, and Outlook
How to Stand Out (Preferred Qualifications):
- Health care experience
- Bilingual proficiency in English / Spanish
- Knowledge of medical terminology
- Basic understanding of Medicare and Medicaid health plans
- Experience working with Medicare and Medicaid populations
