Job summary
Billericay PCN has an opportunity for 2 care co-ordinators to join our primary care network (PCN) on a full time basis, to support our GP practices to assist in re-shaping primary care across our communities. You will be based at one site in Billericay albeit there may be a need to travel across practices in the locality if required.
Main duties of the job
You will enable people to access the services and support they require to meet their health and wellbeing needs, helping to improve peoples quality of life and play a key role in proactively co-ordinating primary care patients to ensure they are provided with the most appropriate service for their needs.
