Job summary
Care co-ordinators provide extra time capacity and expertise to support patients in primary care. They focus on the delivery of personalised care and address local priorities, health inequalities and at risk groups of patients.
Main duties of the job
You will act as a central point of contact to proactively identify and coordinate care for a cohort of patients to deliver personalised care including frail, elderly and those with long term conditions, support shared decision making conversations and develop personalized care support plans based on what matters to the person.
Evidence shows people will have better experiences and well being if their care and support is proactive.

Beaumont Elms Practice is a dynamic and friendly practice across two sites located in Beaumont Street in the Oxford City Centre and Elms Road in Botley. We have around 24,000 patients and a dedicated clinical team of GPs, Nurses, Clinical Pharmacists, a Paramedic, a Physicians Associate and Healthcare Assistants. The clinical team are ably supported by large reception and admin teams.

We are looking for a great administrator with a caring attitude and exceptional communication and organisation skills, including an ability to prioritise, multi-task and work to deadlines.
Experience working in a GP surgery and/or hospital/medical services and previous experience with EMIS will be an advantage. However, these are not essential as full training will be provided. Our primary aim is to recruit the right type of person to the role, a can-do, professional, and caring attitude will be advantageous.
