      If so this is an exciting opportunity for the right candidate to join our dynamic team of existing Care Co-ordinators, working with the GP Surgeries that form Wakefield North Primary Care Network (PCN)

      In this vital role you will support patients by co-ordinating their health and social needs, working to achieve better independence and quality of life

      You will be part of our growing multi-disciplinary additional roles team, who all work together to support our GP practices and their patients.The roles currently include care coordinators, clinical pharmacists, pharmacy technicians, paramedics, health and wellbeing coaches, social prescriber, mental health practitioner and a home visiting nursing team.

      Main duties of the job

      The successful candidate will be part of the GP Surgery team, working independently as well as part of a team to help and support anyone that would benefit from working with a care co-ordinator, such as our frail and elderly or those with a long-term condition, or anyone that would benefit from the support from this exciting and varied role.

      The Care Co-ordinator will proactively (not an exhaustive list):

      identify and work with a cohort of patients to support their personalised care requirements

      consult with patients and family members to bring together all of their identified care needs into a single personalised care and support plan

      engage with patients living in care homes

      support housebound patients

      support patients with long-term conditions and/or work alongside primary care colleagues

      make contact with patients diagnosed with cancer to undertake cancer reviews, and support the cancer screening programme

      identify patients, and support GP practices, with non-medical interventions with referrals to external agencies/partners

      support patients living with Dementia and their carers

      perform health checks on patients that are aged 75 and over

      support patients who are on the Learning Disability Register

      support patients who are informal carers

      work with the pharmacy team to undertake frailty reviews with patients who are housebound

      About us

      Wakefield North PCN consists of six GP practices on the eastern side of Wakefield District (Henry Moore Clinic, Riverside Medical Centre, Tieve Tara Medical Centre, Station Lane Medical Centre, St Thomas Road Surgery and Newland Surgery). We are a friendly and forward-thinking PCN, who support the development of our team members.

      Our PCN is committed to improving the care provided to our patients, reducing health inequalities and raising the standards and quality of our general practices. We are always seeking to work collaboratively with community and voluntary services as well as social enterprise organisations to benefit the health and wellbeing of our patients.
