Job Description

- Be the first point of contact for newly diagnosed cancer patients, including frail or housebound individuals.
- Develop, coordinate, and maintain Personalised Care and Support Plans.
- Liaise with services such as Macmillan, Saint Catherines Hospice, community nursing, and therapy teams.
- Support patients with completing benefit applications (e.g. Blue Badge, Attendance Allowance).
- Coordinate Cancer Care Reviews with GPs and ensure timely referrals.
- Maintain cancer registers, RESPECT forms, and support end-of-life care planning within the Gold Standards Framework.
- Participate in monthly MDT, safeguarding meetings, and cancer screening awareness initiatives.
- Record all interactions and plans using System One.
- Be at the heart of personalised, integrated patient care

We are looking for a compassionate, proactive, and highly organised Cancer Care Coordinator to join our forward-thinking team within the Filey and Scarborough Healthier Communities Network (FSHCN). You will be employed by a lead GP practice on behalf of the PCN and work collaboratively across four practices, serving a combined population of over 31,000 residents.
This vital role supports cancer patients through all phases of their journey from diagnosis to survivorship or end-of-life care by coordinating personalised, timely and holistic care that meets their unique needs.
