      Job summary

      Make a Real Difference in Cancer Care

      We are looking for a compassionate, proactive, and highly organised Cancer Care Coordinator to join our forward-thinking team within the Filey and Scarborough Healthier Communities Network (FSHCN). You will be employed by a lead GP practice on behalf of the PCN and work collaboratively across four practices, serving a combined population of over 31,000 residents.

      This vital role supports cancer patients through all phases of their journey from diagnosis to survivorship or end-of-life care by coordinating personalised, timely and holistic care that meets their unique needs.

      Main duties of the job

      • Be the first point of contact for newly diagnosed cancer patients, including frail or housebound individuals.
      • Develop, coordinate, and maintain Personalised Care and Support Plans.
      • Liaise with services such as Macmillan, Saint Catherines Hospice, community nursing, and therapy teams.
      • Support patients with completing benefit applications (e.g. Blue Badge, Attendance Allowance).
      • Coordinate Cancer Care Reviews with GPs and ensure timely referrals.
      • Maintain cancer registers, RESPECT forms, and support end-of-life care planning within the Gold Standards Framework.
      • Participate in monthly MDT, safeguarding meetings, and cancer screening awareness initiatives.
      • Record all interactions and plans using System One.
