With a portfolio of over 8 million patients, 7500 providers, 30 states, and 400 healthcare facilities, SCP Health is a leader in clinical practice management spanning the entire continuum of care, including emergency medicine, hospital medicine, wellness, telemedicine, intensive care, and ambulatory care.

Why you will love working here:

      - Strong track record of providing excellent work/life balance.

      - Comprehensive benefits package and competitive compensation.

      - Commitment to fostering an inclusive culture of belonging and empowerment through our core values - collaboration, courage, agility, and respect.

      What you'll be doing:

The Annual Wellness Visit (AWV) is a Medicare (primarily age 65+) designed program that encourages beneficiaries to visit their primary care physician at least once per year to assess their health. To adequately assess patient health, the appointment is considerably longer than a routine office visit. A history of medications, a comprehensive list of other physicians the patient may utilize, as well as a variety of screenings (ambulatory, alcohol, depression, etc.) are conducted with the patient. It is not unusual for these appointments to be 30-45 minutes compared to the normal 15-20 minute time allotments used for most appointments.

The Clinical Care Coordinator works as an extension of the physician to conduct a phone interview and complete most of the required AWV information before the actual office visit. The Clinical Care Coordinator conducts the form completion for the patient and the physician’s office to improve patient flow within the office and deliver an enhanced experience to the patient. There are multiple parts to the process:

      • Manage inbound and outbound patient interactions via telephone (primarily) to complete the Medicare Annual Wellness Visit forms.
      • Identification of Patients with an AWV Appointment
      • Abstraction and transcription of the Medical History of the Patient.
      • Communication with the Patient Before the Appointment
      • Utilize customer systems (scheduling platform, reports, etc.) to identify upcoming AWV appointments.
      • Capture data in a clean, concise format according to customer specifications.
      • Assist with the education of upcoming appointment times and requirements for the visit.
      • Follow customer protocols to assess and complete documentation required for services.
      • Observe privacy standards and conform to patient confidentiality (HIPAA) requirements.
      • Other duties as assigned by the program manager/supervisor.

      Who/What we are looking for:

      • Minimum 3 years of experience working in a doctor’s office or hospital setting
      • Minimum 3 years of experience working with patients
