      What to Expect (Job Responsibilities):
      - Serve as primary care manager for high medical risks/needs members with comorbid behavioral health needs
      - Engage members to complete comprehensive needs assessment
      - Develop and implement individualized, person-centered care plans
      - Partner and collaborate with internal care team, providers, and community resources
      - Provide referral and linkage to appropriate services

      What is Required (Qualifications):
      - Master’s degree in social work or bachelor’s degree in social work, psychology, special education, or counseling with at least one year of experience in case management
      - Current, unrestricted independent licensure as a Registered Nurse in Indiana or Licensed practical nurse (LPN) with a minimum of three years of clinical experience with older adults
      - Experience with local behavioral health providers and community support organizations
      - Experience with MS Office, including Word, Excel, and Outlook
      - Experience in long term support services or working with older adults
      - Resident of Indiana with reliable transportation

      How to Stand Out (Preferred Qualifications):
      - 2+ years of experience in long term support services or working with older adults
      - Additional training or certifications in Motivational Interviewing, Stages of Change, Trauma-Informed Care, Person-Centered Care
      - Experience working in team-based care
      - Experience in serving individuals with co-occurring disorders
      - Background in Managed Care
