What to Expect (Job Responsibilities):
- Identify potential diagnosis codes in patient charts for risk assessment visits
- Conduct audits for diagnosis and health maintenance treatment
- Schedule transition of care and assist licensed personnel for comprehensive patient care
- Identify ongoing health maintenance needs and facilitate next steps
- Perform other duties as assigned
What is Required (Qualifications):
- Bachelor's Degree in a healthcare-related field or equivalent combination of education and experience
- No experience required
