Job Description:
Are you passionate about making a difference in patients' lives? We are looking for a dedicated Care Coordinator to join our team and help us provide exceptional care and support to our patients.
As a Care Coordinator, you'll be the guiding light for patients before and after their medical appointments, helping them navigate their healthcare journey with ease. You'll empower patients by educating and engaging them in setting and achieving their health improvement goals. Working closely with IT resources, you'll ensure accurate registry reporting and documentation of evidence-based care, all while adhering to HIPAA regulations to maintain patient confidentiality.
You'll partner with care teams to support patients, providing follow-up contact to ensure they comply with medical recommendations, including medications, lab tests, specialist visits, and more. Your role will also involve overseeing referrals to specialists, ancillary testing, and other enabling services, as well as coordinating access to telephonic medical advice.
Collaboration is key! You'll work with patients, physicians, and care team members to assess progress toward health care goals, and you'll partner with patients and their families to identify and mitigate barriers to treatment. As a patient advocate, you'll negotiate with health plan care managers on behalf of patients and act as the primary point of contact during care transitions between consulting physicians, hospitals, emergency rooms, and skilled nursing facilities.
In addition to providing information and resources to support patients' wellness goals, you'll maintain accurate and timely documentation within the scope of practice and represent the care coordination program on performance improvement teams. You'll also handle general clerical tasks, such as making outbound calls, documenting in EMR, photocopying, scanning, typing, and answering messages in EMR.

Salary Range:
$17.45 - $23.27
