Act as a single point of contact for patients with complex or long-term conditions.

Job summary
A Care Coordinator is a non-clinical member of the practice team whose main purpose is to help patientsespecially those with complex health and social care needsnavigate and manage their care more effectively.
Working closely with GPs, nurses, social prescribers, pharmacists, and other primary care colleagues, Care Coordinators take a proactive approach to supporting patients. They help design and implement personalised care plans, coordinate appointments and referrals, and ensure that patients are connected to the right services at the right time.
This role is central to:
Care Coordinators act as a bridge between the clinical team and the patient, freeing up clinical time while ensuring patients feel supported, informed, and empowered to manage their health.
