      Job summary

      Apollo PCN, based in North Warwickshire, is looking for an enthusiastic and proactive Care Coordinator to join their patient focused, forward thinking team. The successful candidate will work alongside our clinical and non-clinical teams to offer a friendly, caring and supportive experience for patients.

      You will be expected to work with several practices and will report to the Clinical Director.

      We would also consider less than full time working

      Main duties of the job

      Patient focus and support is key to this job role, to provide time and expertise to help guide patients through the healthcare and social care system, to make connections with services and teams, giving opportunity to the patient to be heard and understood, whilst also advocating for them, helping individuals understand their rights and options.

      Patient assessment and identifying specific needs is core to this role, working closely with primary care teams as well as community organisations and other healthcare professionals. Excellent communications skills are paramount with patients, families and healthcare professionals to enhance engagement and quality of life and wellbeing.

      Participate in PCN projects for long term conditions, frailty and others.

      There may be specific project work to support our Practices contractual obligations.

      Holding care coordinator phone and taking patient queries.

      Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner;

      Identify opportunities and gaps in the service and provide feedback to continually improve the service and contribute to business planning;
