Job Description

- Support mental health screenings for individuals navigating same-day access.
- Assist individuals in obtaining needed services in the domains of mental health, social, educational, health, recreational, and related areas to improve child’s functioning.
- Proactively assist individuals to identify barriers regarding access to recommended services.
- Communicate with the members of the individual’s care team to support continuity of care.
- Provide services in a culturally responsive manner recognizing the unique and multiple cultural, medical, psychiatric, and physical needs of each individual.
- Conduct same day access appointments during scheduled hours.
- Participate in regularly scheduled Fraser meetings appropriate to position.
- Support Fraser’s CCBHC process and activities
- Meet and carry out physical requirements of job.
- Regular and timely attendance.
- Provide consultation and communication with internal and external providers regarding shared clients.
- Develop, in collaboration with the Care Coordination Supervisor and Director of Care Coordination, new processes for supporting families in accessing services and resources as appropriate.
- Document, as appropriate, Social Determinants of Health data, mental health screenings and referral information in Fraser’s Electronic Health Record.
- Collaborate with internal and external providers to provide consultation and access for families to receive support.
- Communicate in a respectful and professional manner with others involved with cases, including parents, educational staff, county social workers, probation officers, foster parents, etc.
- Provide additional workshop training and consultation as assigned.

Fraser is seeking a Care Coordinator responsible for supporting same day access and supporting individuals seeking services through Fraser. This position will work with individuals and their care team, as well as in partnership with mental health providers on the Care Coordination team, to successfully complete the initial assessment process and connect with supportive resources.
Available Locations and Schedules:
Location: Fraser Bloomington/Virtual Office & Community-Based
Schedule: Monday-Friday, Full Time with Flexible schedule! Hybrid Setting-50% in office/50% remote
Pay Rate -Starting at $40,000+ annually, depending on Education and Experience
