      What to Expect (Job Responsibilities):
      - Provide case management services to patients, families, and designated caregivers
      - Conduct in-person and remote telephonic assessments for members with Serious Mental Illnesses and Substance Abuse Disorders
      - Develop and monitor multidisciplinary collaboration of services for patients
      - Assist adult patients and their families with personal and environmental difficulties related to medical conditions
      - Participate in the planning and execution of care coordination programs

      What is Required (Qualifications):
      - Must possess a degree in Health & Human Services or a related field from an accredited institution
      - Minimum of 1 year of experience in Long Term Care, Health Plan, and Medicaid
      - Experience in mental health is preferred
      - Ability to conduct assessments and provide case management services
      - Strong interpersonal and communication skills

      How to Stand Out (Preferred Qualifications):
      - Experience in mental health services or counseling
      - Familiarity with community health resources and programs
      - Certification in relevant health and human services fields
      - Bilingual skills can be advantageous
      - Knowledge of Medicaid regulations and policies
