      What to Expect (Job Responsibilities):
      - Perform a variety of casework duties and provide case management services to patients, families, and designated caregivers
      - Develop, participate in, and monitor multidisciplinary collaboration of services for patients
      - Assist adult patients and their families with personal and environmental difficulties associated with medical conditions
      - Conduct in-person and remote telephonic assessments for members with serious mental illnesses and substance abuse disorders
      - Ensure effective communication and coordination of care among healthcare providers and patients

      What is Required (Qualifications):
      - Must possess a degree in Health & Human Services or a related field from an accredited institution
      - Minimum of 1 year of experience in Long Term Care, Health Plans, and Medicaid
      - At least one year of mental health experience preferred
      - Knowledge of case management principles and practices
      - Strong interpersonal and communication skills

      - Experience working with patients dealing with serious mental illnesses and substance abuse disorders
      - Additional certifications in relevant fields such as counseling or social work
      - Familiarity with healthcare regulations and compliance standards
      - Bilingual skills to assist a diverse patient population
