      What to Expect (Job Responsibilities):
      - Perform a variety of casework duties and provide case management services to patients, families, and designated caregivers
      - Develop, participate in, and monitor multidisciplinary collaboration of services for patients
      - Assist adult patients and their families with personal and environmental challenges related to medical conditions
      - Conduct both in-person and remote telephonic assessments regarding serious mental illness
      - Ensure comprehensive support for patients during terminal illness processes

      What is Required (Qualifications):
      - Must possess a degree in Health & Human Services or a related field such as Psychology, Social Work, or Nursing
      - Minimum of 1 year of experience in Long Term Care, Health Plan, and Medicaid services
      - At least one year of mental health experience preferred
      - All degrees must be from accredited institutions recognized by the U.S. Department of Education
      - Candidate must reside in Alexandria, Fairfax, Manassas, Arlington, or surrounding areas

      How to Stand Out (Preferred Qualifications):
      - Experience in mental health services or case management
      - Familiarity with community health resources and support services
      - Proficiency in assessment and evaluation of patient needs
      - Strong communication and interpersonal skills
      - Ability to work collaboratively within a multidisciplinary team
