What to Expect (Job Responsibilities):
- Conduct in-person and remote telephonic assessments of members with serious mental illness
- Provide case management services to patients, families, and designated caregivers
- Develop and monitor multidisciplinary collaboration of services for patients
- Assist adult patients and their families with personal and environmental difficulties related to medical conditions
- Participate in the coordination of care for individuals during terminal illness stages
What is Required (Qualifications):
- Must possess a degree in Health & Human Services or a related field from an accredited institution
- Minimum of 1 year of experience in Long Term Care, Health Plan, and Medicaid
- At least 1 year of mental health experience preferred
- Strong understanding of case management and care coordination
- Ability to conduct assessments and develop treatment plans for patients
How to Stand Out (Preferred Qualifications):
- Experience working in a healthcare setting, particularly in mental health or case management
- Familiarity with community health resources and services
- Additional certifications in mental health or related fields
- Bilingual abilities in English and Spanish or other languages
- Strong communication and interpersonal skills
