What to Expect (Job Responsibilities):
- Perform a variety of casework duties and provide case management services to patients, families, and designated caregivers
- Develop and monitor multidisciplinary collaboration of services to patients as appropriate
- Assist adult patients and their families with personal and environmental difficulties related to medical conditions, including terminal illnesses
- Conduct both in-person face-to-face assessments and remote telephonic assessments of members with serious mental illness
- Work in designated areas within South/Southwest Central Virginia to support community health initiatives
What is Required (Qualifications):
- Degree in Health & Human Services or a related field such as Social Work, Psychology, Nursing, or Counseling
- Minimum of 1 year of experience in Long Term Care, Health Plan, and Medicaid
- At least 1 year of mental health experience preferred
- All degrees must be from accredited institutions recognized by the U.S. Department of Education
- Candidates must reside in the designated geographic areas of South/Southwest Central Virginia

- Experience in mental health services or case management
- Familiarity with community health resources and support services
- Additional certifications in related fields (e.g., counseling, social work)
- Strong interpersonal and communication skills
- Commitment to patient-centered care and community involvement
