- provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients.
- ensure patient health and care planning is timely, efficient, and patient-centred and that all patients receive the best possible care and service.
- support patients preparing for clinical conversations with a primary care professional or following up clinical conversations they have had.
- become an important member of your PCN multidisciplinary team (MDT) identifying patients in need of proactive support ensuring the appropriate support
- ensure support is made available to patients and their carers as well as any needs being addressed.
- work alongside a variety of different people working in the practices and across the PCN and wider community teams including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, paramedics and clinical pharmacists, as well as the wider administrative team.
- Patient Care: Provide high-quality care to patients, including telephone based assessments and follow-ups.
- Collaboration: Work collaboratively with social prescribing link workers, and other health professionals as needed. Work closely with other members of the Primary Care team to deliver the above responsibilities.
- Documentation: Maintain accurate and confidential patient records and documentation.
- Data Management: Work closely with the Data Quality Team to ensure the accuracy, completeness, and consistency of patient data.
- Data Audits: Participate in regular audits and data quality checks to identify and resolve discrepancies.
- Reporting: Support the preparation and submission of monthly, quarterly, and annual claims and reports.
- Meeting Coordination: Co-ordinate and minute Adult and Child Multi-Disciplinary Team Meetings (MDT) and weekly Care Home Meetings.
- Review Scheduling: Invite and book reviews for Serious Mental Illness, Learning Disability, Long Term Conditions and Severe/Moderate Frailty.
- Patient Support: Support patients self-referring and invite and book Carer reviews.
- Carer Support: Provide signposting for Carers, e.g., to the social prescribing team.
- Proactive Initiatives: Take initiative to identify areas for improvement in patient care and data management processes and implement solutions to enhance overall efficiency and effectiveness.
- Additional Roles: Perform any other roles as deemed necessary to support the delivery of care to the Practice and PCN population.
- Knowledge of Regulations: Understanding of healthcare data standards and regulations.
- Strong written and verbal communication skills, need to be able to communicate professionally and effectively with a variety of people at all levels.
- Good IT/computer skills including Word and Excel.
- Motivated by helping people with care and empathy and in delivering person centred support in a non-judgemental way.

Job summary
Harlow South Primary Care Network (PCN) and The Ross Practice in Harlow each have a fantastic opportunity for a care coordinator to join our team on a full time basis, for 37.5 hours per week.
The role will focus on supporting coordination of the patients journey to meet their physical and/or mental health needs, working closely with the 3 practices in the primary care network (PCN).
This role ensures that patients receive coordinated and comprehensive care while maintaining high data quality standards, ultimately improving overall health outcomes.

Harlow South Primary Care Network is a friendly, supportive, well organised and enthusiastic PCN.
GP practices are grouping together and working closely in what are called primary care networks (PCNs) and we are committed to providing traditional family orientated services, working together in the NHS to deliver better patient care on behalf of our 3 GP practices: Lister Medical Centre, The Hamilton Practice, and The Ross Practice. The PCN offices are based in a new purpose built Health Centre in Harlow.
The PCN has over 40,000, patients from three practices. Lister Medical Centre based at Lister House, Staple Tye and The Ross Practice & The Hamilton Practice based at Keats House, Bush Fair.We are a training hub and support learning & development.We use theSystmOne clinical system.
The Practices benefit from easy access to M11/M25, are close to London and Stansted and the attractive Essex and Hertfordshire countryside.
