      What to Expect (Job Responsibilities):
      - Provide telephonic care coordination outreach with both outbound and inbound calls to support preventative health activities
      - Conduct health risk assessments to identify and address member needs
      - Maintain metrics for telephonic outreach and member engagement
      - Participate in team case reviews for collaborative assessment and coordination planning
      - Consult with supervisors to ensure effective delivery of services and accurate reporting of work hours

      What is Required (Qualifications):
      - Minimum educational requirement: High School Diploma or GED equivalent
      - 2+ years of experience working in a healthcare environment
      - 1+ years of care coordination experience
      - Knowledge of medical terminology
      - Ability to work in a highly dynamic environment and complete assessments with members

      How to Stand Out (Preferred Qualifications):
      - Medical Assistant or an Associate's degree in a health-related field
      - Bachelor's degree
      - 2+ years of experience with Medicaid/Oregon Health Plan or other health plans
      - Telephonic outreach or call center experience
      - Strong written and verbal communication skills
