      What to Expect (Job Responsibilities):
      - Conduct outreach to members via phone or home visits to discuss care plans and address questions or concerns
      - Coordinate care activities with healthcare providers and community partners to ensure continuity of care
      - Address member and provider inquiries related to care plans and services
      - Document and maintain accurate member records in compliance with state and regulatory requirements
      - Provide education on available benefits and resources to members

      What is Required (Qualifications):
      - High School diploma or GED
      - 1-2 years of related experience in care coordination or healthcare services
      - For Arkansas Total Care plan: Bachelor's degree in social science/health-related field or 1 year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients
      - Valid driver's license and ability to pass child and adult maltreatment checks and drug screening
      - Must reside in Arkansas or a bordering city with 30% travel required

      How to Stand Out (Preferred Qualifications):
      - Residing in Garland or Hot Springs counties for field-based position
      - Experience working with developmentally or intellectually disabled clients or behavioral health clients
