      What to Expect (Job Responsibilities):
      - Perform case management services for patients, families, and caregivers
      - Develop and monitor multidisciplinary collaboration for patient services
      - Assist adult patients and families with difficulties associated with medical conditions
      - Conduct in-person and remote telephonic assessments in the Roanoke region
      - Foster and monitor collaboration to support coordinated patient care

      What is Required (Qualifications):
      - Bachelor's degree in Health & Human Services or a related field
      - Minimum of 1 year of long-term care experience
      - Strong communication, assessment, and advocacy skills
      - Demonstrated ability to support patients through various stages of care
      - Knowledge of Medicaid and managed care systems preferred

      How to Stand Out (Preferred Qualifications):
      - Experience in discharge planning and managed care
      - Familiarity with LTSS and waiver population case management
      - Knowledge of DMAS and Epic systems
      - Previous experience in community health or public health settings
