      What to Expect (Job Responsibilities):
      - Perform case management services for patients, families, and caregivers, addressing various personal and environmental challenges
      - Develop, participate in, and monitor multidisciplinary collaboration of services for patients
      - Conduct in-person and remote telephonic assessments in and around the Roanoke region
      - Assist adult patients and their families during various stages of care, including terminal illnesses
      - Foster and monitor collaboration among healthcare teams to ensure coordinated patient care

      What is Required (Qualifications):
      - Bachelor’s degree in Health & Human Services or a related field
      - Minimum of 1 year of Long-Term Care experience
      - Strong communication, assessment, and advocacy skills
      - Demonstrated ability to collaborate with multidisciplinary teams
      - Knowledge of managed care practices is preferred

      How to Stand Out (Preferred Qualifications):
      - Discharge planning experience
      - Experience with LTSS, Medicaid, and waiver population case management
      - Familiarity with DMAS and Epic systems
      - Additional certifications in relevant fields
