What to Expect (Job Responsibilities):
- Conduct face-to-face and remote telephonic assessments for Long-Term Services and Supports (LTSS) Waiver members
- Provide case management services to patients, families, and designated caregivers
- Develop and monitor multidisciplinary collaboration of services for patients
- Assist patients and their families with personal and environmental difficulties related to medical conditions
- Support patients through terminal illnesses and end-of-life care considerations
What is Required (Qualifications):
- Must possess a degree in Health & Human Services or a related field from an accredited institution
- Minimum of 1 year of experience in Long-Term Care, Health Plan, and Medicaid services
- At least 1 year of mental health experience preferred
- Must reside in South Boston, VA, or surrounding areas
- Strong communication and interpersonal skills necessary for effective patient interaction
How to Stand Out (Preferred Qualifications):
- Experience in case management or care coordination within a healthcare setting
- Familiarity with Medicaid and LTSS Waiver programs
- Additional certifications in mental health or related fields
- Bilingual skills are a plus
- Demonstrated commitment to community health and patient advocacy
