      £26,554 a year pro rata

We are looking for an organised and patient-focused Care Coordinator (Prescribing Support) to join our Primary Care Network (PCN) team as part of the Additional Roles Reimbursement Scheme (ARRS). This is a non-clinical role focused on improving medication-related support for patients with complex needs, helping to streamline prescription processes and enhance personalised care planning.

You will work closely with clinical staff, including pharmacists and GPs, and support patients in navigating prescription services, especially those managing long-term conditions or taking multiple medications. This is not a pharmacy technician post but one that complements clinical roles through effective coordination and communication.

      Main duties of the job

We are seeking a proactive and patient-focused individual to join our team as a Care Coordinator (Prescribing Support). This vital role bridges patient-facing services and prescription administration, directly supporting GPs and Clinical Pharmacists to streamline medication processes and improve patient care.

      About us

Bramhall and Cheadle Primary Care Network (PCN) is a thriving network of GP practices located in the Stockport area, committed to delivering high-quality, patient care. We aim to provide innovative and coordinated care, enhancing services for patients while promoting preventative healthcare and managing long-term conditions.

Our network is built on strong collaboration between member practices, local health services, and community partners. By working closely together, we aim to streamline patient care and improve access to essential services.

  • About the company

National Health Service (NHS) is the umbrella term for the publicly-funded healthcare systems of the United Kingdom (UK). The founding principles were that services should be comprehensive, universal and free at the point of delivery—a health service based on clinical need, not ability to pay. Each service provides a comprehensive range of health services, free at the point of use for people ordinarily resident in the United Kingdom apart from dental treatment and optical care.
