What to Expect (Job Responsibilities):
- Coordinate care for patients by facilitating access to resources and assisting with referrals and appointments
- Conduct outbound calls and respond to inbound inquiries from patients, providers, and resources
- Complete non-clinical screenings and escalate issues to the Integrated Care Nurse as necessary
- Assist the clinical team in managing the patient care continuum by executing directed tasks
- Communicate with patients professionally and maintain accurate documentation
What is Required (Qualifications):
- High school diploma or GED required
- Minimum of 1 year of experience as a Care Coordinator or in a similar role
- At least 2 years of experience in a professional medical office environment
- Strong verbal communication skills and ability to handle confidential information
- Proficiency in Microsoft Office, Excel, and Outlook
How to Stand Out (Preferred Qualifications):
- 1–3 years of clinical medical office experience
- CNA or MA experience is a plus
- Experience in coordinating care and navigating insurance payers
- Familiarity with Electronic Medical Records
- Prior experience in direct patient care or support
