What to Expect (Job Responsibilities):
- Develop and implement individualized care plans for members addressing their physical, behavioral, and social needs
- Perform face-to-face clinical assessments to evaluate and manage members’ needs
- Coordinate care for high-risk members with chronic conditions or disabilities
- Manage care to ensure cost-effective utilization of health benefits, conducting gap-in-care management
- Work closely with interdisciplinary teams to present and discuss case management treatment plans
What is Required (Qualifications):
- Must possess a degree in Health & Human Services or a related field from an accredited institution
- Minimum of 1 year of experience in long-term care
- Strong knowledge of physical, psychological, socio-cultural, and cognitive patient needs
- Excellent oral and written communication skills, along with strong problem-solving and analytical abilities
- Ability to work independently and collaboratively in a fast-paced remote environment
How to Stand Out (Preferred Qualifications):
- Experience with HealthPlan and Medicaid services
- Familiarity with case management and long-term care systems
- Understanding of regulatory and compliance standards in healthcare
