What to Expect (Job Responsibilities):
- Perform case management services and provide support to patients, families, and designated caregivers
- Develop, participate in, and monitor multidisciplinary collaboration of services for patients
- Assist adult patients and their families with challenges related to medical conditions
- Conduct both in-person and remote telephonic assessments in the Roanoke region and surrounding areas
- Foster and monitor coordinated patient care through strong communication and advocacy skills
What is Required (Qualifications):
- Must possess a Bachelor's degree in Health & Human Services or a related field
- Minimum of 1 year of Long Term Care experience required
- Strong communication, assessment, and advocacy skills to support patients through various stages of care
- Demonstrated skill in fostering multidisciplinary collaboration for coordinated patient care
- Knowledge of DMAS and Epic preferred
How to Stand Out (Preferred Qualifications):
- Experience in discharge planning and managed care
- Familiarity with LTSS, Medicaid, and waiver population case management
- Degrees obtained from accredited institutions as recognized by the U.S. Department of Education
