What to Expect (Job Responsibilities):
- Perform case management services to patients, families, and designated caregivers
- Conduct in-person face-to-face assessments and remote telephonic assessments
- Develop and monitor multidisciplinary collaboration of services for patients
- Assist adult patients and their families with personal and environmental difficulties related to medical conditions
- Support patients through various stages of care with strong communication and advocacy skills
What is Required (Qualifications):
- Must possess a Bachelor's degree in Health & Human Services or a related field
- Minimum of 1-year experience in Long Term Care required
- Strong communication, assessment, and advocacy skills
- Demonstrated ability to foster and monitor multidisciplinary collaboration
- Experience in case management related to LTSS, Medicaid, and waiver populations preferred
How to Stand Out (Preferred Qualifications):
- Discharge planning experience
- Managed Care experience
- Knowledge of DMAS and Epic systems
- Experience in community outreach programs
- Familiarity with behavioral health and public health initiatives
