If so, we’d love to connect with you! We are Mental Health Cooperative (MHC), and we are looking for a Care Coordinator to join our Safety Net Program team! Our Safety Net program provides behavioral health services to consumers who do not meet TennCare (Medicaid) guidelines or experience a reduction or loss of public assistance. You’ll provide continuous treatment team care management services to consumers with severe and persistent mental illness. You’ll ensure consumers obtain Safety Net services, coordinating with PAC staff, Safety Net nurse, and providers. You’ll complete documentation such as application and renewal requirements for Safety Net coverage.
