What to Expect (Job Responsibilities):
- Assess, plan, and implement individualized care strategies for patients
- Identify and initiate referrals for social service programs
- Manage care plans as a single point of contact throughout the continuum of care
- Advocate for patients and families to ensure their needs are fully represented and supported
- Act as a resource for others with less experience
What is Required (Qualifications):
- Bachelor's Degree in Social Work (BSW) or related field
- 2+ years of experience in care coordination, case management, clinical, or community health setting
- Licensed BSW with applicable state licensure
- Intermediate proficiency in Microsoft Word, Excel, and Outlook
- Ability to work any full-time, 8-hour shift during normal business hours including occasional overtime
How to Stand Out (Preferred Qualifications):
- Master’s Degree in Social Work (MSW) with License
- Experience with managing populations with complex medical or behavioral needs
- Experience with electronic charting
- Experience with arranging community resources
- Bilingual fluency in English and Spanish
