What to Expect (Job Responsibilities):
- Perform case management services to patients, families, and designated caregivers
- Develop and monitor multidisciplinary collaboration of services for patients
- Assist adult patients and their families with personal and environmental challenges related to medical conditions
- Conduct both in-person and telephonic assessments for members in nursing homes or their homes
- Ensure quality support for members with chronic illnesses through integrated clinical and behavioral health services
What is Required (Qualifications):
- Minimum of 1 year of experience in Long Term Care, Health Plan, and Medicaid
- Must possess a degree in Health & Human Services or a related field from an accredited institution
- Must reside in Alexandria, Falls Church, Arlington, Annandale, Woodbridge, VA, or surrounding areas
- Experience working with mental health is preferred
- Strong interpersonal skills to communicate effectively with patients and families
How to Stand Out (Preferred Qualifications):
- At least one year of mental health experience
- Knowledge of community health and health education practices
- Familiarity with Medicaid and managed care systems
- Experience in therapy, counseling, or social work settings
- Additional certifications related to health and human services
