What to Expect (Job Responsibilities):
- Perform in-person and telephonic assessments of members in nursing homes or their homes
- Provide case management services to patients, families, and designated caregivers
- Develop and monitor multidisciplinary collaborations of services for patients
- Assist adult patients and their families with personal and environmental difficulties related to medical conditions
- Participate in the care coordination process to improve member health outcomes
What is Required (Qualifications):
- Must possess a degree in Health & Human Services or a related field from an accredited institution
- Minimum of 1 year of experience in Long Term Care, Health Plan, and Medicaid
- At least 1 year of mental health experience preferred
- Strong interpersonal and communication skills
- Ability to work both in person and remotely as required

- Knowledge of community resources and support services
- Proficiency in data management and reporting systems
- Bilingual skills are a plus
