- Communicate through regular touch points both over the phone and in person and updating system notes to accelerate patient discharge
- Drive customer satisfaction and deliver results through execution of effective placements
- Act as the link between discharge teams, local authority, Next of Kin/Families, clients, ward nurses, care providers and service users to facilitate discharge and ensure a seamlessly coordinated service
- Organise caseloads in priority order having fully understood patient needs
