      Job Description

      Shift Schedule: Monday - Friday 8:00AM - 4:30PM

      Location: Baltimore Washington Medical Center

      General Summary:

      Under the supervision of the Manager of Care Management, assist the in-patient Care Management teams by performing non-clinical tasks to facilitate smooth transitions of care throughout the hospital continuum of care. Responsible for issuing regulatory notices, starting and tracking insurance authorization for rehab placements, making follow up appointments as needed, assembling discharge packets, making non-medical transportation arrangements when indicated and also assist with clerical support functions: Prepares documents and correspondence, scanning documents into EPIC, organizes supplies and forms, graphs, etc.

      Reports to: Manager of Care Management

      Qualifications

      • Education & Training:

      High school diploma or equivalency

      • Work Experience:

      1-2 years’ experience in healthcare or related experience with computer experience, Outlook or other email platform, Excel helpful, General knowledge of medical terminology and EMR experience preferred.

      Skills & Abilities:

      • Ability to handle multiple tasks simultaneously
      • communicate effectively with patients, medical staff and team members.
      • maintain confidentiality with patient & team member information.
      • communicate effectively in person, by phone and in writing.
      • attention to detail and follow-through.
      • work independently and take initiative over tasks, Computer literacy in word processing, database & spreadsheets, Microsoft Office, Outlook email
      • strong organizational skills
