      Position Summary


      The Care Management Associate is part of the team that supports our Healthy Aging Support Program, responsible for documentation in our platforms, supporting the Analyst Health Coaches and members throughout the Group Coaching process, making outreach calls to the members to remind them of upcoming appointments, and scheduling the Action Plan Review. Our Care Management Associates demonstrate a highly energetic blend of health care navigator and health plan customer support representative.

      Additional Responsibilities to include but not limited to the following:

      - Effectively meets daily metrics with speed, accuracy, and a positive attitude

      - Accurately and consistently documents attendance for each Group Coaching session and follow up in the member’s electronic record, thoroughly completing required actions with a high level of detail to ensure we meet our compliance requirements

      - Protects the confidentiality of member information and adheres to company policies regarding privacy/ HIPAA

      - Effectively supports Analyst Health Coaches during each Group Coaching session, responding promptly to coaches during the session for support needs, and demonstrating empathy and a sense of urgency when appropriate

      - Ability to be agile, managing multiple priorities and adapting to change with enthusiasm

      - Determined to build strong relationships with peers

      - Demonstrates an outgoing, enthusiastic, and caring presence over the telephone


      Required Qualifications

      - 2 years of experience preferably in customer service, telemarketing and/or sales

      - Experience with computers including knowledge of Microsoft Word, Outlook, and Excel

      - Strong organizational skills, including effective verbal and written communications skills

      Preferred Qualifications

      - Experience with data entry and documentation within member records is preferred

      - Familiarity with basic medical terminology and/or experience working in the medical field

      - Bilingual (Spanish) preferred

      Pay Range

      The typical pay range for this role is:

      $18.50 - $35.29

      This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
