      What to Expect (Job Responsibilities):
      - Perform duties in support of care management and integrated medical management programs
      - Conduct telephonic outreach and care coordination for case and disease management
      - Provide clinical support to ensure effective implementation of medical management programs
      - Collaborate with team members to enhance patient care and outcomes
      - Engage in continuous learning and training to improve care delivery

      What is Required (Qualifications):
      - Bachelor’s degree in a health-related field or equivalent education and experience
      - Completion of Motivational Interview training within 6 months of hire
      - Valid driver's license and proof of auto insurance may be required
      - Minimum of 3 years of experience as a Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), or equivalent in a clinical, managed care, or care management setting

      How to Stand Out (Preferred Qualifications):
      - Experience in managed care services, including knowledge of the Affordable Care Act and Medicare/Medicaid regulations
      - Familiarity with Lean principles and program development in a dynamic healthcare environment
