What to Expect (Job Responsibilities):
- Support core functional areas of care management and integrated medical management programs
- Engage in phone outreach and care coordination for case and disease management programs
- Collaborate with team members to provide clinical support
- Maintain compliance with state laws regarding driving and provide proof of driver license and auto insurance as needed
- Participate in Motivational Interview training within six months of hire

What is Required (Qualifications):
- Minimum of 3 years of work experience in a clinical, managed care, or care management setting
- Required to complete Motivational Interview training within six months of hire
- Must comply with all state laws and requirements for driving, including providing proof of driver license and auto insurance upon request
- Experience as a Certified Medical Assistant (CMA) or Certified Nursing Assistant (CNA) is preferred
- Education in a health-related field or equivalent experience

How to Stand Out (Preferred Qualifications):
- Bachelor's Degree in a health-related field or a combination of equivalent education and experience
- Experience in managed care services, including knowledge of the Affordable Care Act, Medicare, and Medicaid regulations
- Familiarity with Lean principles and program development in a dynamic healthcare environment
