- POSITION SUMMARY
- Responsible and accountable for coordinating the care of patients meeting case management criteria across a continuum of care. Ensures and facilitates the achievement of quality, clinical and cost outcomes. Negotiates, procures and coordinates services and resources needed by the patient and family.

- EDUCATION, CREDENTIALS, TRAINING and EXPERIENCE:
- Minimum Required:
- Care Management Specialist: Graduate of an accredited school of professional nursing, with current license to practice as a Registered Nurse in North Carolina. Thorough knowledge of nursing care process from admission through discharge, gained through several years of clinical experience as a RN
- Psychiatric Care Management Specialist: Graduate of an accredited school of professional nursing, with current license to practice as a Registered Nurse in North Carolina or BSW/MSW from an accredited institution. Thorough knowledge of processes from admission through discharge, gained through several years of clinical experience.

- Preferred:
- Care Management Specialist: Baccalaureate Degree in Nursing and previous experience as a Care Management Specialist. Knowledge of utilization review and discharge planning methods and procedures. Understanding of clinical pathways and disease management models.
- Psychiatric Care Manager: MSW certified as a Clinical Social Worker
