- Receiving patients, consulting with members of practice team GPs, Nurses, Pharmacists, Physios, Admin
- Be able to cover all reception positions as necessary
- Taking messages and passing on information
- Processing personal and telephone requests for appointments, telephone consultations and ensuring callers are directed to the appropriate healthcare professional
- Initiating contact with and responding to requests from patients, other team member and associated healthcare agencies and providers
- Ensure that telephone system is operational at the beginning of each day and switched over to night service and that the answer phone operational at the end of each day
- Book/edit/cancel appointments and recalls ensuring sufficient information is recorded to retrieve medical record
- Deal with home visit requests, carefully noting all details
- Arrange transport to hospital appointments for housebound patients
- Clinical Rooms are checked at the end of each consulting session and left tidy and secure
- Maintain confidentiality of information, acting within the terms of the Data Protection Act and Caldicott guidance on patient confidentiality at all times
- Maintain an awareness of the Freedom of Information Act
- Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data
- The post-holder will manage their own and others health & safety and infection control as defined in the Practices Health & Safety Policy, the Practice Health & Safety Manual, and the Practices Infection Control Policy and published procedures.
- Comply with Practice health & safety policies by following agreed safe working procedures
- Actively report health & safety hazards and infection hazards immediately
- Keeping work and general areas clean and tidy, and using appropriate infection control procedures to keep work areas hygienic and safe from contamination.
- Awareness and compliance with national standards of infection control, hygiene, regulatory / contractual / professional requirements, and good practice guidelines.
- Correct personal use of Personal Protective Equipment (PPE) and ensuring correct use of PPE by others, advising on appropriate circumstances for use by clinicians, staff and patients.
- Reporting incidents using the organisations Incident Reporting System
- Using personal security systems within the workplace according to Practice guidelines
- Making effective use of training to update knowledge and skills

Job summary
We have a vacancy for a Care Navigator / Medical Receptionist at Shaftesbury Medical Centre Leeds LS9 & Church View Surgery LS15
We are a busy two site Practice and are seeking a reliable and experienced Care Navigator / Receptionist to join our friendly team.
We can offer 24 to 28.5 hours per week. Shifts will be split across both of our surgeries in LS9 and LS15
Previous Primary Care / GP surgery experience is preferred but we will also consider applicants with relevant transferable skills.
We are seeking someone with good interpersonal skills who is able to work under pressure and as part of a team of 10 receptionists dealing with all patient enquiries on the phone and in the surgery and via digital online consultation requests from Patients.
Both surgeries are in modern purpose built medical centres and include facilities for rest areas and kitchen facilities with free hot drinks, microwaves and fridges. There is free parking on both sites
Full training will be given to be competent in the role
We can offer between 24 to 28.5 Hours per week for this role
Main duties of the job
Being the 'face' of the Practice to the general public, particularly patients, both in person and on the telephone
Answer calls
Care navigate patients to the most appropriate service
Completing digital online consultation requests on behalf of the Patients for GP services
Booking Nursing appointments
Managing admin tasks
Registering new patients
Ensuring complete confidentiality whilst on reception

You will be joining a well established and friendly team of 10 Receptionists supporting a team of 12 doctors and 7 nurses who serve a population of 17,800 patients
Salary up to £12.46 per hour dependant on experience for 24-28.5 hours a week
