      Job summary

      We are currently looking for a Care Navigator to join our friendly and busy Practice.

      You will be the first point of contact for our patients and will provide a crucial role in providing excellent customer care service.

      The ideal candidate will be organised, efficient and have excellent and strong communication skills and to be able to work well within a team.

      You must be able to communicate with all health care professionals, internally and externally.

      Main duties of the job

      To be the first point of contact for our patients either face to face or by telephone.

      Booking appointments for clinicians.

      Processing patients prescriptions manually and electronically.

      Processing of all emails and electronic letters.

      Distribution of post.

      Scanning.

      Filing of notes.

      Allocation of blood results to GPs.

      Informing patients of results.

      Processing results.

      Tasks from all clinicians externally and internally.

      To use the AccuRx system for patient communication.

      Process patient registrations and change of addresses.

      Processing consent forms and On line access.

      To be able to liaise with all members of the Practice Team.

      Registering patients at the Practice.

      Contacting patients in a polite and courteous way by telephone.

      Processing emails.

      Photocopying.

      Opening and distribution of incoming post from the hospital and external post.

      To be responsible for the opening and closing of the Practice.

      Job responsibilities

      Job Title: Care Navigator

      Responsible to:Reception Manager/Practice

      Job Purpose: To ensure that the reception area of the Practice runs smoothly, provides a high level of service to patients, doctors, nurses and other members of the Practice team.

      Duties and Responsibilities

      Telephone Duties:

      1. Care Navigating patients to the right clinician when booking appointments.

      2. Dealing with queries from Pharmacies, Hospital and other external parties.

      3. Communicating results to patients.

      4. Ambulance transport bookings including blue light Ambulances.

      5. Home visit requests.

      6. Deal with general enquiries, compliments and complaints.

      7. Contacting patients and other NHS staff.

      Reception Duties:

      8. Taking Registration forms from patients and checking they are accurately completed.

      9. Greet and direct patients and visitors.

      10. Monitor reception and waiting room areas including making patients aware of late running surgeries.

      11. Making appointments as a care navigator and directing to right clinician on the front desk.

      12. Opening and dating post and distributing to the right department.

      13. Ensure outstanding queries are explained and handed over to next shift, as necessary.

      14. Respond to needs of doctors and nurses during surgery.

      15. Processing patient request for online services and subject access forms.

      16. Collect payments from patients for non-NHS services.

      17. Accepting samples from patients and making sure adequately labelled.

      Prescriptions:

      18. Processing patient prescriptions paper and online including repeat dispensing.

      19. Resolving prescription queries.

      General:

      20. Be responsible for the opening and closing of the premises in the morning and evening including arming and disarming the alarm system.

      21. Input and extract information from Practice computer system

      22. Always observe health and safety guidelines

      23. General housekeeping (e.g. keeping reception and waiting areas tidy).

      24. Admin duties to include processing tasks, AccuRx system, allocation of blood tests to GP's and processing the acute and medication review list.

      25. Task allocation by Reception Manager or the Practice Manager.
